To provide implementation teams (multi-disciplinary, multi-agency teams that have come together as part of the SW LTC Commissioning Development Programme to implement LTC) across the South West with the evidence base, tools and examples to enable commissioning and provision of an integrated care team to deliver the outcomes of the QIPP LTC Workstream to improve outcomes for patients with LTC.
