Care pathways case study: Workforce and the discharge care pathway in Cambridgeshire
A case study capturing the key workforce findings during a move towards an integrated discharge care pathway for older people with complex needs.
The CfWI has developed an ‘approach to care pathways’ methodology on how to conduct local-level workforce planning along a care pathway, and has been working to test this approach alongside a local area – Cambridgeshire.
The approach to care pathways has eight stages. We have retrospectively applied these eight stages to Cambridgeshire’s integrated discharge pathway to evaluate whether or not the CfWI approach is useful, and how it could be improved.
This case study contains the detailed information obtained about the Cambridgeshire system through carrying out these stages.
This information, along with insight into the challenges that have been faced in Cambridgeshire, as well as the benefits reaped, will be valuable information for other local areas that are moving towards a more integrated discharge process.
