The complex needs of our ageing population are among the greatest challenges to the health and social care system.
There are so many different organisations, disciplines and cultures involved. To improve outcomes, we need to set aside those differences and look again at the way people work together.
Integration between organisations is the key to achieving effective health and social care support for the older population, and workforce planning across organisations will be crucial to the success of this integration.
The CfWI is developing a toolkit to improve the way organisations plan and meet services for people in the community.
